Feedback Forms

Feedback forms are documents used to maximize response in an unobtrusive and intuitive way from clients. The forms are used to provide feedback on performance, quality and general opinions of clients towards an organization. In learning institutions, the forms are used to rate teaching skills, and overall impressions. In hospitals and other service providers like airports they are used to rate the speed and quality of care and customer service. Feedback forms provide a platform in which clients can air their grievances or complaints without fear of victimization or reprimand.

Employers use evaluation feedback forms within an organization, to find out what employees opinion on management style and even how they feel their organizations are aiding their development in both their personal and work lives. Organizations use the information obtained from feedback forms to evaluate their own performance and make any changes necessary or recommended by their clients in order to serve the client better.
