Employee Training Evaluation Form

The employee training evaluation form is issued in order to determine how effectively a training course is and how perfectly it suits the pertaining needs of the trainees. Most of these employee forms are used by the R&D department, in order to hike the efficacy of the training processes. After attending a training program, the employees are required to fill up this form by means of which the program is assessed. First the employee needs to fill in basic details as to which program he attended, who was the instructor and the tenure of the training program. Next he needs to put down in words as to what he/she felt was the objective behind the program.

Next he has to grade the different parameters on a scale of 1-5, where 1 is inadequate, 2 is adequate, 3 is good, 4 is very good and 5 is good. The parameters of the training program differ, however few common parameters are, whether the presentation was organized, was the facilities provided satisfactory enough, will the training be useful to his current job, was the instructed well versed about the topic, would be interested to continue the program etc. Once the employee grades the parameters, he needs to sign it and submit it for evaluation.
