Uniform Statutory Form Power of Attorney
A uniform statutory power of attorney form is used to authorize an agent to act on behalf of his/her principal. This type of form is similar in different states. The form provides guidelines to persons wishing to transfer powers to other individuals. Below is a sample uniform statutory form power of attorney.
Sample Uniform Statutory Form Power of Attorney
I (name), a resident of (address, county, state) appoint (name, address, county, state) as my attorney –in –fact (referred to as the agent) to act in any lawful ways with respect to the following initiated subjects:
To grant all of the following powers initial the line in font of (G) and ignore the lines in front of the other points.
To grant one or more but fewer than all of the following powers initial the lines in front of each power your are granting.
To withhold a power do not initial the line in front of it but need not cross out each power withheld.
