Swimming Liability Release Form

A swimming liability release form is a formal document which is framed when an individual wishes to take part in a swimming competition or a swimming hobby course and takes full responsibility of his/her health status within the swimming complex and during the swimming periods. These forms are drafted by swimming authorities and are to be signed by the participant and a guardian of the participant to give their confirmation for taking full responsibility. These forms release the swimming authorities from any kind of liability.

This swims meet provides the swim time to the participants to practice in the same pool as mentioned above from one month in advance to the competition. I take full responsibility of my well being during this one month and have taken medical permission for participating in the swim meet. I shall be fully responsible for any injury, medical condition or accident caused during the practice sessions and during the competition. Hence I wish to thereby provide my confirmation for the same to the authorities.
