Superintendent Evaluation Form
A superintendent evaluation form is used by the board of directors of an institution to assess the performance of the superintendent running the institution. This is usually triggered in most occasions by complaints arising from say the stakeholders of the institution, and therefore it is deemed necessary for the evaluation to be done.
