Student Peer Evaluation Form

A student peer evaluation form is used to determine a student’s perspective towards the contribution of  fellow members in a class, group project or an activity and use it to construct a base for teaching or instructional base.

KEY

Use the following five point scale:

5 = Excellent

4 = Good

3 = Average

2 = Poor

1 = Weak
