Statement of Understanding Form

Statement of understanding form is a legal document that mentions all the rules and regulations that the person must follow while he or she is initiating any kind of deal, using any particular service or becoming a member of any company, agency or institution etc. It is very important to read the document carefully before signing any deal.

Sample Statement of Understanding Form:

I, ___________ (name of the individual) certify that I have read the attached _______ (card type) card policy and procedures. I understand that the program is designed to improve the administration of Government travel. I am using the card to cover the expenses incurred by me whenever I am travelling officially.

In case I fail to follow the rules and regulations of the card then I can face disciplinary action which can lead to cancellation of the card and losing the authority of using the card.
