Before evicting a tenant, the right thing a landlord should do is issue an eviction form in good time.  Typically, an eviction notice must have the names and addresses of the tenant and landlord, property in question and the reasons and date of evictions.  Here is a sample eviction notice form.

Failure to vacate the premises may result in eviction and/or court proceedings against you.
