Notice to quit form

When you intend on quitting your job, you are required to write a letter of resignation. You need to state that you have no regrets for leaving and try to make sound regretful and unavoidable without any hint of disrespect. You have to sign off with an appreciative note. Remember to include the names of those you will copy to at the bottom.

Sample Notice to Quit Form

Dear Mr._______________________________(Manager’s Name)

I would like to formally notify you of my intention to resign from

_____________________________ as ____________________________in the

________________________department from this _________day of ___________________in the year 20_____. From that day henceforth I will cease to be your employee and will clear my desk.

I wanted to inform you so that you may have time to find a replacement. I appreciate the opportunity I had been given and I wish you and the company much success in the future.
