Notice of Motion Form

A notice of motion form is a legal document used in a court to petition plaintiffs, defendants, attorneys or witnesses to appear in court during the mention of a case.  The notice of motion form is specific although it may vary slightly according to state.  Here is a sample notice of motion form.
