Evaluation Forms



Evaluation forms are used to assess and review different products, services and goods. They can be used by individuals, companies or organizations. Most evaluation forms have questions with specified answers.  In most instances the questions ask the evaluator to rate products, goods or services. By using the rating system, an individual, company or organization can be able to gauge the overall quality of their products, services or goods. Most companies use evaluation forms when they are launching new products, goods or services.

Teachers and lecturers can also use these forms to find out the effectiveness of their teaching methods and materials.  These forms can be general or specific depending on what is being evaluated. The questions can be open or closed. Evaluation forms may require the evaluator to state their name, address and contact information in the event that they need to be called upon again. These forms provide valuable and useful information for organizations.
