Citizenship verification forms are usually required for purposes of clearance for example in an airport or for employment. In addition, if you are enlisted in the army, this form is required to validate your citizenship before deployment.

Certain documents accompany the citizenship verification form. They are a birth certificate and all documents that prove your country of birth and a certificate of citizenship proved by the immigration department. If you are a refugee or a victim of human trafficking, documents such as a letter from the health department must be available.

Ineligible applicants are those who are applying for permanent residency or a student with a non-immigration visa or an employment authorization.
