A power of attorney form is a legal document used to describe the level of authority a Client gives a lawyer in a given matter. This document gives the lawyer power and authority to begin taking action on the matter according to the client’s description and directions. The document below represents a sample of a power of attorney form.

__________________________, as my lawyer and give him the authority to act on my

behalf in the matter described below

The above appointed lawyer has the authority and power to exercise the rights granted to

him in this document and shall take necessary action with effect

This is until the expiry date of this agreement or if either party gives prior notice for
