Appearance Release Form

Often when a celebrity appears on a show that appearance is re-broadcasted and shared among the media. It is also used for promotion of the show in various ways through advertisements in the print media, television and internet. In such cases the channel needs an appearance release form in order to exhibit the same. All the recordings, sound effects, the interview, tape, still images related to the appearance then become the sole and exclusive property of the channel or the producer to be used in whichever fashion he desires. It should however be stipulated that the material would be used in an ethical manner befitting the dignity of the channel and the celebrity.

and its parents, affiliates, subsidiaries and its assigns the right to make use of my appearance. I agree that they may record my appearance and tape and photograph me along with voice, conversation and sounds.

I agree that they can use my voice, likeness and my biographical information for the promotion, sale, advertising and publicizing of the program under consideration. The information given by me in the interview is true to the best of my knowledge.
