Actor Release Form

An Actor Release Form is required to give the producer the authority to use the actor’s images related to the motion picture. Whenever an actor works in a motion picture he or she signs this release form to enable the producer to use the actor’s image or voice for the promotion of the movie. He can use plays, performances, voiceovers, poses etc for the same purpose. It releases the producer from any libel related to the actor’s photograph.

I grant the right to use all the recordings you make of me and my voice and the right to use my name in connection with advertising or promotion of the picture. I release ___________from any suits, actions or demands whatsoever in connection with using my likeness and voice in the picture.

I certify that I am over 18 years of age and fully understand the meaning and intent of the contract.
