When a landlord has to evict a tenant for various reasons, he has to issue a 30-day eviction notice before taking any action.  The eviction notice is a legal binding document and takes the format below.  Although it may vary from country to country, this is a basic 30 day eviction notice form format.

And you are required to vacate the premises on or before ________________ being 30 days from the date of issuance of this notice.  Failure to do this will result in civil proceedings against you for unlawful detainer.
